A routine PSA blood test came back elevated last September. An MRI in April showed a PI-RADS 5 lesion. A biopsy on Friday, May 15 confirmed prostate cancer — Grade Group 2, with adverse pathology features that take active surveillance off the table. A June PSMA PET scan showed no spread. Over the summer, pathology re-reads — including one by the pathologist who created the grading system — walked back those adverse features, and both radiation oncologists dropped the hormone-therapy recommendation. It's now a hormone-free choice between surgery and radiation, and I'm picking a path this week.
